- Preschool Registration Form
- Immunization Form
- Emergency Consent Form
- Walking Trip Policy Acknowledgement
- Picture Permission Form
- Child Questionnaire
- Copy of your Child’s Birth Certificate
- Any Photo that shows your child’s face
